The client plays an integral role in the treatment process and how we administer case management, where goals and objectives are tailored to their unique needs, strengths, abilities, and preferences. These goals are articulated using the client’s own language, and upon request, they have full access to their individualized plan.

Each client is assigned a dedicated case manager who takes charge of optimizing their daily activities, facilitating program integration, connecting them with community resources, advocating for their needs, and assisting in developing social and relationship skills.

In addition, educational activities and referral services are available to support the enhancement of overall life functioning and improve the quality of life.

Regularly scheduled meetings are conducted with each client to review their progress toward their individual goals, ensuring that the Individual Plan is updated and revised as necessary.
